Who We Are

Access Care Partners Inc. is a private, non-profit agency founded in 1974, and celebrating its 50th anniversary in 2024, that serves Holyoke, Chicopee, South Hadley, Granby, Belchertown, Ludlow, Ware and other surrounding communities.

What We Do

As part of a national network of Aging Services Access Points and Area Agencies on Aging, Access Care Partners Inc. provides home and community supports to older adults, persons with disabilities, individuals with chronic conditions, and caregivers.
